Interim Market Director of Case Management Needed in Pennsylvania Healthcare System
The Position
- An Interim Market Director of Case Management is needed to support case management operations across multiple hospital locations, with various bed capacities.
- This role will report directly to the Market CFO and work closely with the CMO in the oversight of approximately 20 FTEs.
- The leader will implement necessary changes to improve metrics, focusing on care coordination, discharge planning, and compliance with CMS guidelines. They will provide education and guidance to a younger group of providers, aiming to enhance their discharge processes.
- Ideal candidates will have previous director-level experience in hospitals with over 150 beds, expertise in EPIC, InterQual, and Conditions of Participation, and a strong background in discharge planning and transitional care management will be critical.
- Must be available to start within 2-3 weeks of acceptance.
Requirements
- Bachelor’s degree required.
- Active RN license required.
- Experience with acute care, LTAC, SNFs, and TCU required.
