Seeking an Accountable Care expert for a new Director of Population Health opportunity with Yavapai Regional Medical Center located in Prescott, Arizona

The Position
  • The Director of Population Health will be responsible for leading and coordinating the efforts to move Yavapai Regional Medical Center (YRCMC) and its member organizations toward a value-based payment environment with an emphasis on population health management
  • Working closely with the Executive Medical Director, physician networks and Board, this inspired leader will work to align current and future strategy and operations of the North Central Arizona Accountable Care Organization (NCAAC) toward its goal of enhancing population health programs and patient experience while reducing per capita costs
  • Key goals include building strong relationships with member hospitals and associated networks, driving data reporting and innovative technology enhancements for the department and developing an effective cost reporting process
  • Bachelor’s degree required, Master’s preferred
  • 5+ years of leadership experience within accountable care, population health management and/or case management setting is required

The Organization 
  • Yavapai Regional Medical Center (YRMC) is a state-of-the-art healthcare system with two acute care hospitals, a network of primary and specialty care clinics, outpatient health and wellness centers, cardiac diagnostic centers, and outpatient medical imaging centers
  • North Central Arizona Accountable Care (NCAACO) is a physician governed, joint venture between YRMC and Summit Regional Medical Center (SRMC) responsible for redefining the way that healthcare is coordinated and delivered in Yavapai, Coconino and Navajo counties
  • With over 500 providers and already serving over 15,000 Medicare beneficiary lives today, the goal of NCAAC is to improve the overall health and wellness of the current and future residents in the community
